Faith Formation Registration 2025-2026

To register your child for the Parish Faith Formation Program,
for the 2025-2026 academic year,
please print out the form found below
(or pick up a copy found in the church sacristy or the rectory/parish office).

Then, with the completed form, please enclose a copy of your child's baptismal certificate (required!).
(We will not accept any registrations without the baptismal certificate.)
You can either drop it off at the rectory or sacristy, or mail it to:

In-house registrations will take place on Sundays, August 10-17-24, 2025, from 9-11 AM in the Church Hall.
